This is a fully Work From Home Opportunity (WFH)
Salary: $24/hr to $26/hr DOE
Responsibilities:
Provide customer service to potential patients/families, professional referral sources, and Accent Care staff to effectively and efficiently coordinate referrals, respond to inquiries about hospice and Palliative services and effectively coordinate visits to provide patient care. Serve as a member on the After-Hours Team for building and maintaining provider relationships.

  • Receive professional referrals (from discharge planners, physician offices, facility staff) and obtain the critical referral information in the form easiest for the referral source (electronic/fax notification, phone call, Hospice Care Consultant (HCC) notification, etc.) to assure timely response and data entry into the Electronic Medical Record (EMR).
  • Receive potential patient/family inquiries regarding Hospice services. Obtain and input into EMR, the basic information regarding the patient status in order to describe how Accent Care may assist, with a goal of scheduling a visit to initiate the relationship.
  • Schedule visits in coordination with local field staff to assure the appropriate team member makes the initial contact (depending on patient condition and family situation) to meet the professional referral source, and/or the patient/family initial service needs.
  • Communicate with referral sources, attending physicians, Accent Care staff members, and other agencies as needed, to coordinate initiation of hospice services. Notifying the Palliative Teams of any palliative referrals, so those visits can be coordinated by the appropriate team.
  • Maintain regular communication with all involved in the sign-on, consent and admission assessment phases to assure seamless coordination of information and services.
  • Provide solutions (a visit from an Accent Care representative) for callers seeking Hospice information. Maintain accurate EMR documentation and referral statistics.
Qualifications:
  • Minimum 1 year Hospice experience and/or demonstrated commitment to hospice philosophy of care, preferred.
  • Excellent customer service/verbal and written communication skills.
  • Strong organizational skills and computer knowledge.
  • Demonstrated ability to work closely with a variety of people while effectively using problem solving skills.
  • Demonstrated commitment to the AccentCare Hospice philosophy of care, values, mission statement and organizational culture.
