Job Summary
The Social Services Associate I is a key component in supporting existing and emerging initiatives in social services. This position’s main responsibility is to support the department in program building activities and implementing initiatives/projects that address the social determinants of health affecting our members. The Social Services Associate I also provides general support to the Social Work Team, including but not limited to: collecting data to determine eligibility into special programs; data management activities to track referrals and outcomes of special programs; produce reports for the various initiatives/projects.
Duties
Provides program support for the department to ensure successful development/ implementation of existing and emerging initiatives. (20%)
Receives referrals for special projects/initiatives, screens referrals for eligibility, collects and maintains database using L.A. Care’s systems of records (such as: AVES, CCA, MEIS,/QMEIS, E-connect, and Navitis), as well as outside systems such as HMIS System, and CHAMPS System. (30%)
Analyzes data and produces reports to the department and the leadership team to help manage the projects service delivery. (5%)
Assists with care coordination between L.A. Care Clinical Team, Participating Physician Group (PPGs), Hospitals and community partners. (5%)
Serve as a liaison to the community stakeholders, providers, governmental agencies, and other contracted providers. Provides technical assistance and acts as initial point of contact for CHAMPS and HMIS help desk. (20%)
Coordinate and assist with logistics for assigned tasks/activities, develop written communications, presentations, as needed. (10%)
Performs other duties as assigned. (10%)

Experience
Required:
At least 0-2 years of experience in Health Care developing reports, tracking data and providing program support.
Preferred:
Working with adults, seniors, person with disabilities, homeless/chronically homeless individuals, and/or dually diagnosed individuals; Knowledge of resources in Los Angeles County, particularly homeless services and resources.
Skills
Required:
Strong customer service.
Excellent verbal and written communication skills.
Strong analytical, conflict resolutions and persuasion skills.
Proficiency with Microsoft Office including Outlook, Word, and Excel.
Ability to relate to patients, families, co-workers, the multi-disciplinary team and community agencies in a professional and sensitive manner.
Preferred:
Bilingual in one of LA Care Health Plan’s threshold languages is highly desirable. English, Spanish, Chinese, Armenian, Arabic, Farsi, Khmer, Korean, Russian, Tagalog, Vietnamese. .
